That time Dean cut his cast with an angle grinder because Sam took off without him and took baby.  

He was on the phone with Bobby about Sam running off just
before this scene and Bobby was like “Look, he
says he’s okay, so give it a couple of days. Just till you get the cast off,
then hunt him down. Till then, we’ll both just keep calling.”

And Dean was like, “alright”, but then immediately after he hangs up we get this scene. Protective big brother mode is always on with Dean.
